the ltmodem drivers are useless for

  Class 0780: 11d4:1805 Communication controller: Analog Devices SM56
PCI modem
     Primary device ID:  11d4:1805
 Support type needed or chipset:        Motorola_not_supported

and slmodemd will not support this chipset either.
Read InfoGeneral.txt  about alternative hardware.
