Chris,

For modem in  00:1e.3	slot:
8086:266d Modem: Intel Corporation 82801FB/FBM/FR/FW/FRW
8086:266d
1179:0001 Subsystem (known to have a supported AgereSystems chip)

This is your primary problem:
[ 23.737971] ACPI: PCI interrupt for device 0000:00:1e.3 disabled
-----------
read Bootup.txt about possible fixes. Additionally we recently had a
reboot that changing from Ubuntu to Mandriva Linux cured the disabled
problem

The guidance you would have gotten with the modem enabled can be
output anyway with:
./scanModem fake 8086:266d  1179:1001

The essence is:
 Download from http://linmodems.technion.ac.il/packages/smartlink/
 the package SLMODEMD.gcc4.1.tar.gz having a compiled slmodemd. Unpack
under Linux with:
        $ tar zxf SLMODEMD.gcc4.1.tar.gz
 and read instructions therein. But briefly, the modem is setup with command:
        sudo slmodemd -c YCANADA --alsa modem:1
 reporting dynamic creation of ports:
        /dev/ttySL0 --> /dev/pts/N   , with N some number
 Read Smartlink.txt and Modem/YourSystem.txt for follow through guidance.

But until the modem is enabled on bootup, the slmodemd cannot serve.
