This is presumably being done through the action of bootup
sl-modem-daemon scripts,
doing the equivalent of
$ sudo slmodemd -c USA /dev/slamr0

After bootup, try the test:
$ sudo  wvdialconf  /etc/wvdial.conf

If the modem is thus found,
$ sudo gedit  /etc/wvdial.conf
putting in your dialout info and adding a line
  Carrier Check  =  no

Then try to dial out with:
$ sudo wvdial.

If
$  sudo  wvdialconf  /etc/wvdial.conf
fails, there is a possibility that SUbsystem 104d:8175	has a Conexant chip.
Fake a Conexant chip with:
./scanModem test 8086:284b 17aa:20ac
and follow the directions for hsfmodem installation.

Report back presently.
