Do you have an independent info source that there is a Conexant modem
on your System, possible in the Subsystem of
 PCI slot       PCI ID          SubsystemID     Name
 ----------     ---------       ---------       --------------
 00:11.6        1106:3068               Communication controller: VIA
Technologies, Inc. AC'97 Modem Controller
----------------
Return any info from Microsoft diagostics. See Softmodem.txt for instructions.

   The hsfmodem drivers as is cannot support either of your modems,
and they may interfere. So your first step is
$ sudo hsfconfig --uninstall

Then reboot and run
$ sudo ./scanModem
to assess if the hsfmodem package removal has cleaned up the problem with
 PCI slot       PCI ID          SubsystemID     Name
 ----------     ---------       ---------       --------------
 00:11.6        1106:3068               Communication controller: VIA
Technologies, Inc. AC'97 Modem

Also after hsfmodem software removal return the output of
$ lspci -s  00:11.6 -nv
to provide the Subsystem info.

If not a Conexant type, this modem would normally be supported by the
snd-via82xx-modem driver,
 which Ubuntu installs by default
plus the slmodemd helper.

When you report back, provide some additional information on the
 PCI slot       PCI ID          SubsystemID     Name

 00:0e.0        11c1:044e       1235:044e       Communication
controller: Agere Systems LT WinModem
