The 14f1 of 14f1:5423  is a Conexant VendorID . So this is definitely
a Conexant softmodem under the 8086:266d  modem controller.

But your Primary problem is the
[   16.846505] ACPI: PCI interrupt for device 0000:00:1e.3 disabled
This MUST be corrected FIRST.  Read the options in Bootup.txt

Once the IRQ is enabled you can use the free hsfmodem package from Dell,

which requires compiling under each new kernel,

 or pay once for the pre-compiled drivers from www.linuxant.com/drivers
