The setup command sequence is:
$  su  - root
#  slmodemd -c USA  --alsa hw:0.1
or possibly
#  slmodemd -c USA  --alsa hw:0.6

scanModem could no output this information because of:

> hsfich: probe of 0000:00:1f.6 failed with error -5
After you have run the hsfmodem uninstall, shutdown and detach any
non-essential USB devices.  Then boot into Linux and then rerun
scanModem
If may write out the needed slmodemd command line.

Be sure to run the UNINSTALL for the hsfmodem package you have installed.
Its drivers are known to interfere with slmodemd sevices.
