the archives relate that the 1025:003d	has an Agere Systems chip.
Thus in principle:
Download from http://linmodems.technion.ac.il/packages/smartlink/
the package SLMODEMD.gcc4.tar.gz having a compiled slmodemd. Unpack
under Linux with:
       $ tar zxf SLMODEMD.gcc4.tar.gz
and read instructions therein. But briefly, the modem is setup with command:
       sudo slmodemd -c YOUR_COUNTRY --alsa modem:1
reporting dynamic creation of ports:
       /dev/ttySL0 --> /dev/pts/N   , with N some number

However this will not suffice because:
ACPI: PCI interrupt for device 0000:00:1f.6 disabled

Read Bootup.txt about possible fixes. Do
$ dmesg > dmesg.txt
Send dmesg.txt to discuss@xxxxxxxxxxxxx
We might be able to deduce the hardware problem. From
Modem interrupt assignment and sharing:
10:      25056          XT-PIC  uhci_hcd:usb1, uhci_hcd:usb2,
uhci_hcd:usb3, ehci_hcd:usb4, Intel 82801DB-ICH4, ohci1394, eth1,
yenta

all this device sharing might be the problem.  Detach any USB device
and remove any PCMCIA cards before bootup.  After bootup, rerun
scanModem to see if the output has improved.
