Having researched this (i.e. played with KATE till I understood exactly 
what is wrong), it appears that the bug is simple: when you open a file 
in KATE from the filemanager it doesn't open in the current default 
session.  The current default session should be "Default Session" unless 
another session has been saved as default.  Because the file is not 
opened in the default session, it is not added to any session.

This bug needs to be filled.
