Good that I have enough accounts to play with ;-) 

Gmail wizard doesn't ask you. 
For regular account you'll have to uncheck box to create separate folders. 

I tried workaround creating subfolders in KMail, and it is too much work on 
each account. Additional problem is that all folders are treated the same 
way, for instance sorted alphabetically, have same icons (can be changed), 
etc.  
It is possible to create filters and designate default account to reply from, 
but it is all work that Thunderbird user doesn't have to do. 

BTW, practical question, did anybody checked source code, or at least, 
configuration files? 

What is the difference between IMAP and POP settings in kmailrc? 
I have no IMAP configured, so I can't see by myself.

The solution could be as easy as different template for UI, or even just some 
bit in kmailrc.
