As an acceptable workaround I position my icons with "align to grid" checked, and when I'm happy with the icons, I uncheck "align to grid". Also "line up horizontally" (as well as vertically) are unchecked. The problem appears especially when there is an icon close to the panel; KDE seems to think that there is not enough space for it and rearranges everything if one of the automatic alignment options is set.
