We run with altnamespace on but we do set unixhierarchysep off. Neither of those options have any effect on the problem we're experiencing.
However, I did briefly set altnamespace to on, and this seemed to have an initial positive effect; the . LIST "" * command returned a full list of folders, and the INBOX suddenly had the HasChildren attribute instead of HasNoChildren, but this threw off the view of all folders within email clients such as Thunderbird and caused some serious confusion, so I had to turn if back off. Even after it was off we had to repair damage that was caused by email clients suddenly being confused about folder locations and subscriptions.
We're still stuck in the same situation as originally reported.
