Are you saying that the sort order could be wrong or that the incorrect sort order could be causing the other issues I'm seeing? Should I just remove those DELETED folder entries? They don't seem to exist anywhere in the file system. With your mention of bugs though, I went through the bugs fixed from 2.14-2.16 and I see bug 2685. That sort of sounds like what I'm experiencing. It sounds like I should upgrade before spending too much more time on this.
