One might be tempted to argue, indeed, that what is broken is the proliferation of policies, written procedures, and other administrative hurdles that large and mature organizations tend to build for themselves. These start as an effort to make rules so that the activities of the organization are consistent and low-cost, but the imperfection of the rules tends to cause effort to be spent on perfecting the rules. That attention to rule-perfection takes attention from the very activity the rules were meant to facilitate. I leave it to the gentle reader to evaluate to what extent the current state of affairs matches that description.
