Volume keys problems with T400

Hello,

with latest kernel 2.6.28 and thinkpad_acpi 0.21-20081111 the volume
keys does not work correctly on my Lenovo T400 (7UET49WW).

I can mute the sound by pressing the mute key:
$ cat /proc/acpi/ibm/volume
level:          15
mute:           on
commands:       up, down, mute
commands:       level <level> (<level> is 0-15)

and unmute by pressing volume up or down key:
$ cat /proc/acpi/ibm/volume
level:          15
mute:           on
commands:       up, down, mute
commands:       level <level> (<level> is 0-15)

The mute status has no effect when a audio device (eg headphones) is
pluged in (the sound is always on).

The sound level can't be changed with the volume up and down keys. With
"echo up > /proc/acpi/ibm/volume" and "echo down >
/proc/acpi/ibm/volume" the level changes in  /proc/acpi/ibm/volume, but
has no effect.
