You can use RADIUS based routing in conjunction with an SQL backend.
Maybe SQLAuth module can also do that (you need to try the later cvs probably).

Do you know of any way to re-write the E164 or add a prefix dependent on the results of an SQL lookup? It has to be done for each call.
