Do you really need all of 3 account modules "required"?

There are different possibilities: required, optional, sufficient,
alternative.
Look at the manual:
# required - if the module fails to log an event, the final status is set to
failure and the event is passed down to any remaining modules....
And what do you think what the final status for account will it be, if there
is empty StartQuery in the section?
> [SQLAcct]
> Driver=MySql
> Host=localhost
> Database=billing
> Username=root
> Password=root
> MiniPoolSize=5
> StartQuery
> StopQuery=INSERT INTO CDRS VALUES (null, NOW(),'%g', '%n', '%d', '%r',
> '%c', '%s', '%u', '%{CallId}', '%{ConfId}', '%{setup-time}',
> '%{connect-time}', '%{disconnect-time}', '%{caller-ip}', '%{callee-ip}',
> '%{Dialed-Number}', '%{Called-Station-Id}') TimeStampFormat=MySQL
And you've recieved this:
> 19:48:58.859 2 sqlacct.cxx(238) GKACCT SQLAcct failed to store

and this:

> Failed to log event 1 for call no. 1

> failed for call no. 1
