You can use GnuGk + FreeRADIUS + SqlBill - it allows alias control AND assignement, so your endpoint only needs to know its h323id, e164 can be assigned from the database.

For each client, the gnugk configuration file should contain: - h323id - e.164 <== very important - password
When the client try to register, if its parameters don't match against the gnugk config., then its request is rejected.
Note: the client IP address is not fixed (dhcp, mobile user, ...).
Is it possible to do it with gnugk?
