Unless one is not looking for real time billing one can create some sort
of parsing program that reads in the cdr files and dumps them into a
database every x min or so. And just use the mysql for real time
monitoring reports.
I think this would probably be the most accurate and reliable.

Also I've been using mysql accounting for a while and haven't had any
problems, to me it seems very accurate, but again mysql hasn't crashed
on me yet. :)
