Both alternative and sufficient rules stop accounting processing (on success), so I would suggest rather something like this:
[Gatekeeper:Acct] FileAcct=required;stop RadAcct=sufficient;start,stop default=accept
Otherwise, logging is stopped just after successful RadAcct update. Check also directory/file permissions. Your log contains very little details, so it's hard to say what's going on exactly.
