I have a patch that perfom this task and it seems that
it works very well. But now we have more important
issues to resolve, so it will take some time before it
will be available in the cvs, as it needs to be updated
to the current cvs state.

General idea is to route based on h323-redirect-ip-address
and/or h323-redirect-number/Called-Station-Id attributes. 
This way we can have rerouting/rewritting. In addition, Calling-Station-Id
can be used to screen the source number.
