I am not sure if this is a question or a statement, but if you are
asking when it uses case II, then that is when you set GKRouted=1 and
Enable=1 in the gatekeeper.ini file in the [RoutedMode] and [Proxy]
sections respectively.

EG: 

...
[RoutedMode]
GKRouted=1
H245Routed=1
...
[Proxy]
Enable=1
