Try the newest gk version - it works as you would expect.
The problem is that your billing system does the wrong thing
- if the call is not to be accepted, Access-Reject has to be sent.
Also Cisco docs say that Access-Accept can only carry
h323-return-code=0. But many billing system vendors tend
to ignore this.

But... you have a bigger problem - you are using Quintum billing
and it is sending Cisco attributes with Quintum vendorId, so these
are not even recognized by the gk. If you want to have this working
you need to modify the source code to send/check VSA attributes
with vendorId = 6618 instead of vendorId = 9.
