Try running "pulseaudio -k && sleep 5 && pulseaudio -vvv" which kills the pulseaudio daemon, sleeps for 5 seconds then start's it with verbose mode then you can run "pulseaudio -k && sleep 5 && pulseaudio -D" to daemon-ized it once again.
