I'd argue that conceptually /opt and /usr should be seen as the same thing for RPM: i.e. during update time both are writable and outside of updates they are both to be considered read-only from RPM's PoV. Hence it should be fine too put the rpmdb in /usr/ too in that case. Or with other words: if RPM is used to install something in /opt, it should be fine to require that both /usr and /opt are writable then.
