When his happens, I see 100% kswapd in top; but no change in swap
pagein/pageout at all. In fact, minimal I/O.

That lead me to this bug:

And also to this recent kernel work, not yet landed and which may not
be related (since there's not much I/O in my case) to this problem;
but might help somewhat with the other thread on earlyoom and
unresponsive systems.
