which is why you have a second DVD iso that has the extras - and not put them on the first.
Those who want both at install time simply download and install two iso's.
Of course - using something jigdo would be even better - as you could have one jigdo image for just core, that creates just a core iso, and second jigdo that creates a single dvd with core and extras. (and jigdo's to create the CD's as well - all mirrors would need are the packages and the jigdo files - works quite nicely for Debian)
