[PATCH v4 03/17] drm/vram: switch vram helpers to the new gem_ttm_bo_device_init()

This allows to drop drm_gem_vram_mmap_offset() and
drm_gem_vram_driver_dumb_mmap_offset(), the default
gem function works just fine.
