dm_ioband - BUG: unable to handle kernel NULL pointer dereference at (null)

Hello!

I have subj error on two nodes with difference in several minutes.

Nodes configuration: SLES11SP1 with 2.6.32.36-0.5-xen kernel, and
dm-ioband-1.14.0 module.

After this error both nodes become inaccessible by network, with no
reaction on keyboard and needs to be power cycled:
