New PR Checklist for Ceph Upstream Contributions

Hello everyone,

Recently, the PR checklist for Ceph upstream contributions was updated to include a few more items: https://github.com/ceph/ceph/pull/43290

Most of the items are self-explanatory, but the main idea is that one checkbox from each category must be selected in order to pass the new "Pull Request Checklist / Verify (pull_request)" test.

To address some confusion about which option to select for the "Teuthology" category, the "Completed teuthology run" checkbox should be checked only once your PR has undergone a Teuthology test run that has been reviewed and approved. Otherwise, leave that checkbox blank and use the "needs-qa" label in cases where your PR has been reviewed and is ready to go through Teuthology. Select the "No teuthology test necessary (e.g., documentation)" option when a QA run is not needed.
