That's exactly what I'm suspecting.
He's using 2.10, the hotdisk is a dead giveaway.
And he's trying to use userspace RBD.
Jon, whenever reporting bugs (which in this case are totally unrelated to
CEPH) all relevant version information is helpful.
In your case distro (I suspect debian testing or backports), kernel, etc.
I have ganeti 2.9 and 2.10 running with backports and testing and RBD in
kernelspace w/o problems.
While a qemu version with userspace RBD support is in unstable, it hasn't
made it to testing or backports yet (last I checked was Friday).
