Can you just use one of the /dev/disk/by-<something>/<identifier> symlinks?

Eg
/dev/disk/by-uuid/153cf32b-e46b-4d31-95ef-749db3a88d02
/dev/disk/by-id/scsi-SATA_WDC_WD10EACS-00D_WD-WCAU66606660

Your distribution should allow for such things automatically, and if not you should be able to add some udev rules to do it.
