I used the current release of GRML today for system recovery purposes (Grub needed to be reinstalled).

After booting it from a USB drive, I ran both BRLTTY and Speakup from the shell prompt. I probably could have enabled them during the boot procedure, but I was in a hurry. All worked as expected, including detection of the braille display.

I think slint can fill the bill for you.  You can put slint on a flash
drive if you need to do that and have it install for you.
