When a phone initiates an SMS (MO) it tries to establish a direct connection to another device, it will initiate an HLR look-up etc, and if it cant deliver to the final device it will go to the destination user's SMSC for later delivery. For fixed networks, there's usually a connection into an SMSC, which will then deliver into a phone on it's own network or try a phone on another network and if not it get's queued into the other networks SMSC.
