Are you running mod_proxy_html 3.0.0 without any ProxyHTMLLinks
(or including the proxy_html.conf provided)?

Any 3.x without ProxyHTMLLinks will not do anything useful.
In 3.0.0 it will also segfault, due to a known bug fixed in 3.0.1.
