Sorry, I'm not aware of stand alone LDAP enrollments, but it should be
easy for you to create a web based one with PERL or PHP. Have you
searched the web?

For administration, I love http://phpldapadmin.sourceforge.net/

Otherwise, I've seen user self enrollment bundled with a specific
application (like a Content Management or Blogging or Bulletin Board
System).

In my experience, creating users in LDAP is a function of a structured
process where you import information from a data source (like HR systems
or a user database) rather than allow free-form entry by users. The
value of LDAP is that it is an open structure for its schema and
attributes, but that the data in it is consistent with standards.
