I'm looking at http://httpd.apache.org/docs/2.0/install.html but it seems
mainly aimed at people who are already very familiar with the system
(indeed, who wouldn't need the information on that page). The trouble is
that if you're not familiar with it, then even a small omissions in the
instructions can leave you lost.

For example, there's no section on recompiling a build that's already on
your machine.  This is a dedicated Web server I'm leasing;

The install instructions are only for someone installing apache from
source obtained from httpd.apache.org. Otherwise, you need to get
instructions from whomever you obtained apache from. There are so
many different possibilities for install locations/configurations/etc
that it would be impossible to document them all in the main docs.

As to your original problem, note that MaxClients defaults to 256; see
http://httpd.apache.org/docs/2.0/misc/perf-tuning.html
and
http://httpd.apache.org/docs/2.0/mod/prefork.html
