Many servers, One domain:

One way is to assign more than one IP address to the domain name (try
nslookup of www.msn.com - they currently have two IPs assigned). I think
a browser will then choose the IP randomly when sending the request. If
each server has access to the same data, it's transparent to the user.

Otherwise, you need a load-balancing proxy in front of the webservers
(see: http://e-docs.bea.com/platform/docs81/deploy/loadbal.html for
background)

Not sure I entirely understand this... Do you mean how do you keep
/usr/local/www on server1 synchronised with /usr/local/www on server2?
(ie, two physical disks)

One solution is, don't. Put the data in one place and then have one of
the servers mount the disk remotely (see http://nfs.sourceforge.net/).

Otherwise, you need some disk mirroring (see
http://people.freebsd.org/~rse/mirror/)
