Well, since this part of the Autoconf manual does not deal with any
Automake functionality, Makefile.am would be out of place in the diagram
which may be found on
<http://www.gnu.org/software/autoconf/manual/html_node/Making-configure-Scripts.html>.

So, in packages using Autoconf but not Automake, Makefile.in files are
typically hand-written, thus both authored by the package developer, as
well as files used at configure time.  I think that is what the left
resp. right columns in that diagram are supposed to show.  I agree that
the diagram is confusing that way, though; not sure what would be a good
way to fix that.
