I don't check the settings that often to notice if it happens after
Firefox updates, or has no obvious trigger. Though I notice they're
enabled by default on a new installation (something I object to).
That's where writing /etc/firefox/policies/policies.json before
installing Firefox can help, and could be a good thing to check on a new
install along with firewall rules and other configuration.
I also use multiple Firefox profiles, and having the policies file in
place can help save time and prevent errors. Better than having to check
settings manually or copy a user.js file into each profile directory.
