Thank you all for your advice, which has taught me a lot. But I'm ashamed to admit that I found the cause of my problem - after going back to basics and re-discovering xev and showkey - lay in a dirty AltGr key on my Thinkpad T60, which only responded when struck hard in the top right-hand corner. If it had been any other key (except perhaps for the 2 Windows keys) I would have noticed; but I don't think I have ever pressed AltGr except when trying to print the € symbol.
