PulseAudio in-/output device confusion

It's OK for output. You can ignore input at this time, Phonon doesn't really 
support capturing yet.

The "PulseAudio Sound Server" entry is the ALSA pulse device. The entry 
called just "PulseAudio" is native PulseAudio (so it's preferred).

A future release of Phonon will no longer show all those ALSA devices when 
PulseAudio is in use, but instead offer a list of devices which can be 
accessed through PulseAudio. But this is work in progress (by Colin 
Guthrie).
