I'll try to do more debugging. FWIW my usb host controllers are:

1002:4374
00:13.0 USB controller: Advanced Micro Devices [AMD] nee ATI IXP SB400 USB Host Controller (prog-if 10 [OHCI])

00:13.1 USB controller: Advanced Micro Devices [AMD] nee ATI IXP SB400 USB Host Controller (prog-if 10 [OHCI])
1002:4373
00:13.2 USB controller: Advanced Micro Devices [AMD] nee ATI IXP SB400 USB2 Host Controller (prog-if 20 [EHCI])

The hardware device is a usb-bluetooth headset. lsusb:

Bus 003 Device 002: ID 046d:0a29 Logitech, Inc. 
Bus 001 Device 001: ID 1d6b:0002 Linux Foundation 2.0 root hub
Bus 002 Device 001: ID 1d6b:0001 Linux Foundation 1.1 root hub
Bus 003 Device 001: ID 1d6b:0001 Linux Foundation 1.1 root hub

When the bug occurs, i.e. when the audio mysteriously stops playing (I
forget if the bluetooth connection breaks -- I think it does, but I will
double-check), when I pull the usb dongle out, it is recorded in the
syslog:

kernel: usb 3-1: USB disconnect, device number 4

But it is not detected at all when I plug it back in. So that does
indicate an underlying usb host problem?

I'll try to get more info. I don't suppose there are any patches I can
try applying in the meantime?
