As Clemens pointed out, the quirk I suggested earlier shouldn't be
necessary.

Can you first try to obtain the full dmesg output from when plugging in
the device? The necessary steps are:

- unplug the device
- run 'sudo dmesg -c'
- plug in the device
- wait for at least 2 seconds
- run 'sudo dmesg -c' again and post the output of this last command

I see you posted some dmesg output at
http://www.linuxmusicians.com/viewtopic.php?f=6&t=11068 but I think that
was incomplete.
