ICE 1724 requires reboots

I've been pulling my hair out for a week chasing down an 
intermittentsound problem. I could usesome help on what to try next.

I have a Vantec UGT-S110 sound card using the ICE1724 chip.  On boot-up 
I either get good sound or none at all.  If it doesn't work, nothing I 
can do will make it work.  If it works, itworks fine until the next time 
I need to reboot.  It seems to work on about 40% of the boots.

I'm running this on Ubuntu 12.04 with all the latest maintenance.  I'll 
include kernel information, etc, in the aadebug output later in this post.

As a diagnostic, I uninstalled PulseAudio. This didn't alter the problem 
at all, so PA isn't part of the problem. For testing purpose, I use 
aplay and a known good .wav file.

I've checked connections, replaced, speakers, etc., so I think those are 
pretty well out of the picture.  I've reseated the card.

One complication is my video card, which supports sound output via HDMI. 
I've blacklisted the appropriate module to make sure ALSA isn't trying 
to push sound out the wrong card. After blacklisting, "aplay -L" and 
"aplay -l" don't list the HDMI audio output, so I *think* that's out of 
the picture.

Below are two listings of "aadebug" (a script I found in an ALSA 
troubleshooting FAQ).  The first one was taken when the card works.  The 
second one was taken when the card doesn't work. Most of the differences 
are in date stamps or interrupt counts, but the broken one also 
indicates that snd_seq_dummy is loaded.  I don't know why that's loaded, 
or if it's a root cause or just a symptom. I have another one aadebug 
output that seems to show a failure when snd_seq_dummy wasn't loaded, 
though I'm wondering if that was a case of experimental error.

I'm happy to supply other information if you tell me what you need. Does 
anyone have any suggestions or advice on what to try next?
