Separate Device for Headphones?

I've spent days trying to google for useful information on how to set
up alsa to do what I want, with no luck, so now I'm hoping someone
here can tell me.

I have an (as far as I can tell) fully functioning and setup sound
system using alsa. My microphone and speakers work, as do my
headphones. Currently I only use the microphone and speakers with
skype, via pulseaudio, but I would love to get rid of pulseaudio as it
crashes all the time. The thing is, pulseaudio creates a headphone
device for me, and a microphone device and they work great (when they
work at all).

If I just remove pulseaudio, then skype asks me to choose alsa devices
for input and output. There is no alsa microphone device, and no
separate headphones. In addition the microphone is always on, and
relays everything to the speakers so if there is a loud noise, I get a
feedback squeal. I have tried having skype use my hw:0,0 as input and
it does seem to sort-of work, but the sound picked up from the
microphone is very soft and distorted, not clear as it was with
pulseaudio.

So, since I've had everything working with other software, I'm pretty
sure its just a simple matter of configuration to get alsa to provide
headphone and microphone devices for me. I just don't know how to do
it.

I am running 64-bit Debian SID with a 2.6.30-2-amd64 kernel on an Asus
P5K-E/Wifi motherboard, with an AD1988B sound chip. I'm using the
snd_hda_intel driver.

Here's my current asound.conf:

I'm attaching the output of both 'amixer contents' and
/proc/asound/card0/codec#0 as they are both quite long.

I'll happily provide any further information needed in order to figure
out how to create these new input/output devices.
