I'll answer what I can.
1. Not sure of your question...... here so I'll leave this along
2. This is hardware dependant.. many/most onboard digital outputs are fixed at 16 bit 48 khz.... devices like the M-Audio delta series are flexible and can be set via software application (i.e. envy24control)... If you are using jackd then it is set by jackd but your hardware needs to be capable of the bit and sample rates that you have set.
3. So long as you are not using any plugs in asound.conf(system level) or .asoundrc (user level) to my best understanding no conversion is performed...... an ALSA developer maybe able to shed more light on the issue.  
4. to check for bit-perfect output is easy but you need the right equipment.. you need to be able to record the output stream from the digital outpun into a system (hard disk recorded or another computer) that you know records a bit-perfect stream.. extract the resulting wave file and compare the original with the recorded file...
