I always managed to see them sooner or later by restarting things (the application, alsa, my computer, etc). I also had the impression, that they only show up after they have been used at least once. If you find out, what exactly does the trick, please tell me. What's more difficult is to remove them! The value of the controls is stored in /etc/asound.state. In some (most?) distributions, this is done every thime you shut down your computer. If you remove a control, its state in /etc/asound.state will not be removed, so mixers will continue to display the control. What you have to do is delete that file, and then restart alsa without shutting it down correctly (so the control states will not be saved again). I always pushed the reset button of my computer, but maybe killing the process works just fine, too. Again, if you found out something precise, please post it here or write an article in the wiki.
