PulseAudio should use the ALSA device "default" (not "dmix" or "hw:0"). It's a bug if Pulse defaults to hw:0. Something must be blocking the soundcard. Next time you get this problem post the output of "fuser /dev/dsp" and "fuser /dev/snd/*".
