kernel-2.6.18-164.el5 rhel 5.0
xfs_progs ver: 2.9.4

I haven't updated the controller on this server, it just panicked while
doing I/O

 [<f894585c>] do_cciss_request+0x397/0x3a3 [cciss]
looks like the controller was wedged there?
