   The only foolproof way is to hack the server code.  In
xc/programs/Xserver/dix/cursor.c, have CheckForEmptyMask always set:

  bits->emptyMask = TRUE;

  This will remove it permanently and server-wide.
  This is a common request now-a-days.  Maybe somebody ought to make a
commandline option to do this?  Any takers?
